Hemodialysis solutions: Indications for use

Treatment of acute and chronic renal failure, as a replacement solution for hemofiltration and hemodiafiltration and as a dialysis solution for hemofiltration and hemodiafiltration. Accusol K+0 is specifically for patients with hyperkalemia and Accusol K+4 for patients with hypokalemia.

Dosage and application

For use in hemofiltration, hemodialysis and hemodiafiltration. The amount of replacement solution for adults is determined by the rate of ultrafiltration and can be changed by the doctor according to the clinical condition of the patient. Chronic renal failure from 7 to 35 ml / kg / h. Acute renal failure from 20 to 35 ml / kg / h. Dialysate may be prescribed by the doctor according to the clinical condition of the patient.

Side effects

From the procedure: nausea, vomiting, convulsions, hypotension, increased blood clotting, infection, and air embolism.

From the solution: metabolic alkalosis, electrolyte and / or fluid imbalance, hypoglycemia, hypo- and hypervolemia, hypo- and hypertension.

Precautionary measures

To control the purity of the Nikkiso Accusol solution and particles during renal replacement therapy, pre- and post-dilution lines should be carefully checked every 30 minutes. If particles are found, the Accousol solution and line should be replaced immediately and the patient carefully examined. Regularly monitor fluid balance, acid-base balance, glycemia and potassium. Alkalosis may increase if the inter-chamber septum is not open and the patient is supplied with solution only from the small chamber "B".

Contraindications

Metabolic alkalosis. Composition: hypokalemia if there is no addition of potassium (Accusol K+0, Accusol K+2), hyperkalemia (Accusol K+4). Kidney failure with increasing hypercatabolism when urythmic symptoms can no longer be alleviated by hemofiltration; bleeding at the vascular access site or high risk of bleeding due to systemic anticoagulation.

Interaction with drugs

Of particular note are the possible interactions between this course and other treatments. Blood concentrations of other drugs may change during hemodialysis, hemofiltration, and hemodiafiltration. In patients taking cardioglycosides, it is necessary to control the level of potassium in the plasma in order to avoid arrhythmias caused by hypokalemia. Vitamin D and preparations containing calcium may increase the risk of hypercalcemia (for example, calcium carbonate acts as a potassium chelator). Supplemental replacement of sodium bicarbonate increases the risk of metabolic alkalosis.

Overdose

Hypervolemia and electrolyte imbalance may occur.
